Immunization Records

Need to view your current immunization record, update your child’s record, or book an immunization?

Explore the options below to get started.

Please note: Immunization records are not automatically updated from doctor’s offices. It is the responsibility of parents and caregivers to ensure their child’s records are kept current with Public Health.

Immunization records that have been reported to public health are available online through Immunization Connect Ontario (ICON). With ICON you can view, submit and review what immunization your child needs or will need in the future.

Your immunization record will be available online if:

  • Your immunization record has been provided to Public Health.
  • Your health card number has been provided to Public Health.

If you cannot find your immunization record online, please contact your health care provider to get a copy of your record.

We offer immunization clinics for people without a local Health Care Provider. These clinics are available by appointment only. Book an appointment online or call us.

You will need to provide:

  • your child's name and date of birth
  • name of the vaccine(s) 
  • date the vaccine was given

Bring the record to a Lakelands Public Health office.

Exemptions

In Ontario, children must be up to date on their immunizations or have a valid exemption on file with public health to attend school or licensed childcare. The Immunization of School Pupils Act outlines which vaccines are needed for children in school and the Child Care and Early Years Act outlines which vaccines are needed for children in licensed childcare.

Please note: Immunization exemptions require proper documentation and approval from Lakelands Public Health.

Exemption for Religious or Conscience Reasons: 

If you are choosing not to immunize your school-aged child for religious or conscience reasons, you will need to: 

  1. Watch the vaccine education video: English video  |  French video
  2. Complete Vaccine Education Certificate Survey to receive a certificate.
  3. Complete a Statement of Conscience or Religious Belief Formand have it signed and stamped by a commissioner of oaths, notary public, or a justice of the peace. There may be a cost to have the form signed and photo ID is required. Or, check with the Service Ontario location near you, as they may do it free of charge.
    • Note: Affidavit forms that have been altered e.g., additions or information crossed-out, are not accepted.
  4. Submit the certificate and the completed, unaltered Statement of Conscience or Religious Belief Form to Public Health one of the following ways listed above under 'Update'.
  5. Keep a copy for your records. 

Exemption for Medical Reasons: 

If your child cannot be vaccinated for medical reasons, you will need to: 

  1. Have your doctor or nurse practitioner complete and sign the Statement of Medical Exemption Form. 
  2. Submit the completed Statement of Medical Exemption Form to Public Health one of the ways listed above under 'Update'.
  3. Keep a copy for your records. 

Exemption for Religious or Conscience Reasons: 

If you are choosing not to immunize your child for religious or conscience reasons, you will need to: 

  1. Complete a Statement of Conscience or Religious Belief Form and have it signed and stamped by a commissioner of oaths, notary public, or a justice of the peace.  There may be a cost to have the form signed and photo ID is required. Or, check with the Service Ontario location near you, as they may do it free of charge.
    • Note: Affidavit forms that have been altered e.g., additions or information crossed-out, are not accepted.
  2. Submit the completed, unaltered Statement of Conscience or Religious Belief Form to your child’s daycare.
  3. Keep a copy for your records. *Please note an education session is not required and this form will expire when your child begins school.
  4. The daycare will submit a copy to Lakelands Public Health.

Exemption for Medical Reasons: 

If your child cannot be vaccinated for medical reasons, you will need to: 

  1. Have your doctor or nurse practitioner complete and sign the Statement of Medical Exemption Form. 
  2. Submit the completed Statement of Medical Exemption Form to your child’s daycare. 
  3. Keep a copy for your records. 
  4. The daycare will submit a copy to the Lakelands Public Health.

Exemption for Religious or Conscience Reasons: 

If you are choosing not to receive immunizations for religious or conscience reasons, you will need to: 

  1. Complete a Statement of Conscience or Religious Belief Form. Ensure you select that you are completing the Statement of Conscience or Religious Belief Form. form as "An Individual". Have it signed and stamped by a commissioner of oaths, notary public, or a justice of the peace.  There may be a cost to have the form signed and photo ID is required. Or, check with the Service Ontario location near you, as they may do it free of charge.
    • Note: Affidavit forms that have been altered e.g., additions or information crossed-out, are not accepted.
  2. Submit the completed, unaltered Statement of Conscience or Religious Belief Form to your employer. 
  3. Keep a copy for your records. 

Exemption for Medical Reasons: 

If you cannot receive immunizations for medical reasons, you will need to: 

  1. Have your doctor or nurse practitioner complete and sign the Statement of Medical Exemption Form
  2. Select that you are completing the form as "An Individual"
  3. Submit the completed Statement of Medical Exemption Form to your employer.
  4. Keep a copy for your records.
