Reporting an Outbreak?

If your facility has a confirmed or suspected outbreak, please notify Lakelands Public Health as soon as possible.

Choose ONE of the following reporting methods:

Instructions for the Online Outbreak Notification Form

When you select the Outbreak Notification Form, you will first be directed to a demographics page. Please follow the instructions provided on that page:

  1. Enter the required information anonymously.
  2. Select "No Health Card Number."
  3. On the next page, choose "ID - Outbreak Assessment."
  4. Complete the online outbreak reporting questionnaire.
