Reporting an Outbreak?

If your facility has a confirmed or suspected outbreak, please notify Lakelands Public Health as soon as possible.

Instructions for the Online Outbreak Notification Form

When you select the Outbreak Notification Form, you will first be directed to a demographics page. Please follow the instructions provided on that page:

Enter the required information anonymously.
Select "No Health Card Number."
On the next page, choose "ID - Outbreak Assessment."
Complete the online outbreak reporting questionnaire.

The RSV and COVID-19 consent forms may be used as guides. Any facility using these forms must remove the Lakelands logo and privacy statement before use.

Tuberculosis

Residents of long-term care and retirement homes, as well as employees and volunteers, are at increased risk of TB exposure and TB infection. This quick reference guide outlines essential TB screening requirements.
