Position Summary:
The Health Systems Strategist is responsible for advancing coordinated, integrated health system initiatives across programs, partners and health and social service sectors.
Working within an equity-informed approach the incumbent will support organizational priorities by strengthening relationships with internal and external health system partners, leading strategic initiatives and improving alignment across services to enhance population health outcomes.
This position operates at a systems level, supporting planning, implementation, evaluation, and continuous improvement activities that advance collaboration and improve integration with the health system.
Essential Duties and Responsibilities:
Health System Strategy & Planning
- Lead or support the development, implementation and evaluation of health system strategies aligned with organizational and Ministry priorities.
- Conduct environmental scans, needs assessments and jurisdictional reviews to inform strategic planning and service alignment.
- Translate system-level priorities into actionable plans, recommendations, and deliverables.
- Support leadership with briefing materials, reports and presentations related to health system planning and integration.
System Integration & Alignment
- Identify opportunities to improve coordination and integration across internal programs, services, and workflows.
- Support integration initiatives across public health, primary care, acute care, and community health.
- Facilitate alignment of policies, procedures, and practices to support consistent and coordinated service delivery.
- Contribute to quality improvement initiatives focused on system performance, access, equity, and outcomes.
Partner Engagement & Liaison
- Act as a liaison between the organization and health system partners (e.g., hospitals, primary care, community agencies, regional bodies).
- Build and maintain collaborative relationships with internal teams and external partners.
- Participate in relevant organizational committees and working groups.
- Support consultation and engagement processes with partners and communities.
Project Leadership & Change Management
- Apply change management principles to support adoption of new processes or system changes.
- Support documentation, knowledge transfer, and sustainability planning.
Evidence, Analysis & Reporting
- Analyze qualitative and quantitative data to inform decision-making and system planning.
- Support performance measurement and reporting related to system initiatives.
- Apply health equity, population health, and evidence-informed frameworks to planning and evaluation work.
Experience and Qualifications:
- University degree in a relevant field (e.g., Public Health, Health Administration, Health Policy, Social Sciences, Nursing, or related discipline) and a Graduate degree (e.g., MPH, MHA, MSc, or equivalent).
- Demonstrated experience in health system planning, strategy, integration, or partnership development.
- Experience working in and/or across multiple sectors, including public health, healthcare delivery, community services, or broader health systems.
- Strong analytical, project management and community engagement skills.
- Knowledge of provincial health systems, legislation, and accountability frameworks.
- Experience applying health equity, population health, or quality improvement frameworks.
- Familiarity with change management and systems-thinking approaches.
- Demonstrated research and analytical skills to gather, analyze, and synthesize data and develop recommendations, as necessary.
- Excellent written and verbal communication skills, including report writing and presentations.
- Superior time management and organizational skills.
- Ability to work independently, collaboratively, and effectively within multidisciplinary environments.
- Demonstrated commitment to diversity, equity, and inclusion through continuous development, modeling inclusive behaviours, and proactively managing bias.
- Successful completion of a Criminal Records Check and Vulnerable Sector Check.
