Share Your Feedback with Lakelands Public Health

Lakelands Public Health is rolling out a new brand, and with it, we are launching a community survey to hear directly from you. This survey is designed to gather initial feedback about how community members understand public health and the role Lakelands Public Health plays in keeping our communities healthy.

Your input is important. The survey includes an open text box where you can provide feedback on how Lakelands Public Health can better meet the needs of the community. This is an opportunity to share your thoughts and help shape how we deliver programs and services across the County of Haliburton, the City of Kawartha Lakes, Northumberland County, and the City and County of Peterborough.

Your Privacy Matters

The protection of your privacy is very important to us. This survey is completely anonymous and is being conducted by a third party. No personal identifiers are collected, and all results will be reported back to Lakelands Public Health as grouped data only.

Please do not include your name, address, phone number or other personal identifiers in any of the comment boxes. Anonymous excerpts from comments may be included in a summary report, but any identifying information will be removed by the third party before results are shared.
